For your Veterans, one of the easiest ways to coordinate their return to community living is to contact their VA social worker. He/She can assist with determining eligibility for VA home benefits. Most Veterans can meet the clinical criteria for home care assistance following LTC placement. The following link outlines some of the available benefits - VA Geriatrics and Extended Care. The programs can include a homemaker, HHA, skilled nursing, a SW, and potentially more as needed. As with many areas of the VA, the important part is to start early in the discharge planning process. Contact the local VA as soon as the initial admission assessment information is completed and it's determined that the resident is a Veteran and desires to eventually return home.
